Class Applications and Forms You Need

These forms are important steps in the process whether you are taking EMT for the first time or taking CEU programs. Please follow the instructions and complete the forms.

EMT Initial Class Training Fund Eligibility Form

If you are taking EMT for the first time, you must complete and submit the EMT Training Fund Certificate of Eligibility. This form must be signed by an Officer with the organization you are joining or are a member of.

 

Print it by going to the FILE menu tab on your browser and then hit PRINT. Fill it completely out and mail it to us with your registration for the class.

EMT Recertification Class Training Fund Eligibility Form

If you are taking the Recertification Class, you must complete and submit the EMT Training Fund Certificate of Eligibility for Continuing Education. This form must be signed by an Officer with the organization you are joining or are a member of.  The State OEMS office requires an original signature for each section of the Recertification Class series.

NJSFAC 20th District Volunteer Ambulance Elective CEU Tuition Exemption Form

 

There is no tuition fee for an EMT who is a member of a 20th District of the NJSFAC Volunteer Ambulance Corps.  You must submit a separate signed 20th Dist NJSFAC Volunteer Ambulance Squad Elective CEU Tuition Exemption Form for each course.

 

EMT's who are not members of a NJSFAC 20th District Volunteer Ambulance Corps must submit payment.  The New Jersey EMT Training Fund no longer covers Elective Continuing Education Units.

EMT Integrated Refresher Programs

The EMT Integrated Refresher Program is designed to review the skills, knowledge and critical thinking needed to provide optimum prehospital care.  This entire program totals 24 CEUs/hours, but is divided into an "A" Course, a "B" Course and a "C" Course for 8 CEUs/hours each Course.  These can count as either required or elective credits based upon the continuing educaation the EMT or EMT-B has already completed.  Each A, B or C class may be taken individually, but all 3 must be completed prior to the student's EMT card expiration date.

 

These classes include the expanded scope of practice materials in accordance with the 2009 National EMS Education Standards and meets the NREMT requirements of a "Transition" program.  This course may also be used by reciprocity candidates, individuals with expired NJ EMT certifications, Physicians, PAs, RNs or APNs to obtain current NJ EMT certification.

 

A current Professional Rescuer CPR card is required.  There will be both skills and written evaluations each class, starting with the very first session.  Students are strongly advised to read a current initial or transition textbook prior to the first session. 

 

Tuition: Free to members of a New Jersey Volunteer agency with a signed NJ DOHSS OEMS EMT Training Fund Certification of Eligibility for Continuing Education Courses form.  Signatures must be originals; mechanical reproductions will not be permitted.  A separate form with an original signature is required for each A, B and C class.  Students must bring their current original CPR card and their current original EMT card to class with them (no copies).  Click here to download the EMT Continuing Education Eligibility Form.  The cost is $80.00 for each course (A, B or C) for each student who is not eligible for the EMT Training Fund.

NOTE: EMT-Bs with expired cards, and Doctors/Nurses taking this course for the first time must take the NJ EMT Certification Written Exam.  NJ OEMS policy has been changed and anyone with an expired NJ EMT certification, REGARDLESS OF THE EXPIRATION DATE, may regain his or her EMT certification by having a current valid Professional Rescuer CPR certification, successfully complete the Recertification Program (all 24 CEUs) and pass the NJ EMT Certification Exam with a score of at least 70%.

EMT Transition Refresher Program

The EMT Integrated Refresher Program is 8 hours in length and is intended to update the currently practicing NJ EMT-B to the new knowledge and skills of the Emergency Medical Technician (EMT) described in the 2009 National EMS Education Standards. It is intended for current EMT-Bs who have recently successfully completed a CORE 13 recertification course. NJ EMT-Bs may assign the 8 CEUs earned from this course to either the Core or Elective category, but not both. This course will present the new skills and knowledge that some providers may have been exposed to in practice and in continuing education, but were not part of the “old” EMT-B curriculum. Successful completion of this course permits the EMT-B to immediately operate under the revised EMT scope of practice; and to be able to recertify as an EMT. This course also meets the NREMT requirements for transition from EMT-B to EMT. This course is acceptable for recertification in place of the 24 hour courses, if you meet the above criteria.

EMT Elective CEU Programs

The following Elective CEU classes are currently on our schedule.  Please log on to www.njoemscert.com to register for any of these classes.  Also please select the link to the class you are interested in taking, and print and complete the form, and bring it with you to class. 

When registering at www.njoemscert.com you must first log into the website with your EMT ID number and the password you have created (password is case sensitive and must contain at least 1 letter, 1 number and 1 special character).  Select the class you wish to register for using either the Course Catalog to locate the class, or the Communication tab (Communication, then select ILT Calendar, then select the correct date of the class and scroll through the time until you locate your class).  Once you select the correct class click on 'VIEW' and then scroll to the bottom of the specific class you wish to take.  Click on 'Preview My Schedule' and the program will display all of the courses you have registered for.  Scroll to the bottom of the list and find the new class you have selected and then scroll all the way to the right.  You will see the word 'Register' in dark blue.  Click on that and you will be registered for the class.  HOWEVER, if your EMT ID number begins with a '6' you will be unable to log into this website.  All students are strongly urged to maintain careful records of your courses, especially for those who have EMT ID numbers beginning with a 6.

 

Tuition is as listed on each class flyer.  The EMT Training Fund no longer covers Elective EMT CEUs.  No tuition fee will be charged to any EMT who is a member of a 20th District NJSFAC member agency; you must submit the NJSFAC 20th District Member Agency Elective CEU Exemption Form.  Click here to print this form, complete it in full and bring it with you to class.  Book fees are not included in this exemption.



Bloodborne Pathogens/Hazard Communications (formerly Right-to-Know):

The BBP/Haz-Com class meets the yearly OSHA requirement for BBP and meets the two year OSHA requirement for Hazard Communications.  This is a 3 Elective CEU class being offered on Monday June 5, 2017 at 7:30 PM, at the Englewood Volunteer Ambulance Corps. 7 Jay St, Englewood, NJ  07631 (Bergen County).  The fee for this class is $15.00; there is no cost for members of eligible 20th District Squads.  Please register on-line at www.njoemscert.com, and click here to print this application form.


 

CEVO 4 (Coaching the Emergency Vehicle Operator):
The CEVO 4 Ambulance Course adapts National Safety Council proven defensive driving techniques to the unique nature of ambulance work, training drivers in a six hour class to safely navigate in traffic in both emergency and non-emergency situations under severe time constraints and stress.  This class has been redesigned and now includes new materials such as responding to an "Active Shooter" scene and the "Move Over Law".  This 6 hour, 6 Elective CEU class is being offered on Saturday February 18, 2017 at 9:00 AM, at the Englewood Volunteer Ambulance Corps, 7 Jay St, Englewood, NJ  07631 (Bergen County).  The fee for this class is $65.00, which includes the book and testing fee.   There is no fee for members of eligible 20th District Squads.  Please register on-line at www.njoemscert.com, and click here to print this application form.

Hazardous Materials Awareness Recertification:

The Haz-Mat Awareness recertification class is the OSHA approved Refresher course required yearly for all EMS responders.  This is a 3 hour, 3 Elective CEU class being offered on Thursday January 12, 2017 at 7:30 PM at the Ho-Ho-Kus Volunteer Ambulance Corps, 55 Sycamore Ave, Ho-Ho-Kus, NJ  07423. Understanding Diabetes:

Bandaging and Splinting Review:

This course provides an overview of the mechanics of bleeding and shock and the techniques used in controlling bleeding.  The musculoskeletal system and care of possible broken bones will be reviewed as well.  Techniques to control bleeding and stabilize broken bones will be practiced by  the students.  Bandaging and Splinting Review is a 3 Elective CEU class and is being offered on     at 7:30 PM, at the    . CBRNE (Chemical, Biological, Radiological, Nuclear, Explosive) Awareness:

This is a four hour, 4 Elective CEU course that will be presented as a classroom lecture. It is designed for first responders who operate at the awareness level. The lecture will familiarize students with the recognition of weapons of mass destruction, terrorism and the defensive actions they may use in a response to a terrorist incident. Clinical Assessment for the EMT:

The Clinical Assessment for the EMT class is a 3 Elective CEU class being offered on      at 7:30 PM, at the       NJ  07432. Crew Chief 101:

The Crew Chief 101 class is a 3 hour, 3 Elective CEU class being offered on       at 7:30 PM, at the      . EMS for the Special Needs Patient:

EMS for the Special Needs Patient reviews the emotional and behavioral aspects of patients, whether psychiatric or substance abuse in nature, and of patients with cognitive or social problems, including autism.  Communication and physical care (treatment) of these patients will be discussed.  EMS for the Special Needs Patient is a 3 Elective CEU class.

EMS - Is There an App for That?:

This class is a 1.5 hour, 1.5 Elective CEU class which reviews the multitude of smartphone apps available, and attempts to highlight the apps which would be most helpful to EMT students in preparing for their certification exams, and attempts to highlight the apps EMTs are most likely to use in rendering patient careand/or in emergency situations. 


Psychiatric & Behavioral Emergencies:

This class is a review of Psychiatric and Behavioral Emergencies EMTs may be called upon to handle and will review criteria for use of restraints.  This is a 2 hour, 2 Elective CEU class and is being offered on          , at the        (Bergen County).  The fee for this class is $15.00; there is no cost for members of eligible 20th District Squads.

Spanish for the EMT:

The Spanish for the EMT class is an 8 Elective CEU class.  The cost for this class is $30.00 for non-eligible EMTs.

First Responder/Emergency Medical Responder

First Responders are often the first trained person to arrive at the scene of an emergency. You will be expected to take appropriate action and provide care for injuries or sudden illness until more advanced medical personnel arrive. This course prepares you to fulfill this role as a First Responder. Students will practice basic skills in small groups.

 

The 2009 National EMS Education Standards has changed the name of this course to Emergency Medical Responder.  This course should be taken by Police Officers, Firefighters, Rescuers, Office of Emergency Management Personnel, Recreation Coaches, Scout Leaders, Teachers, and any other persons who wish to be prepared.

We are offering an EMR Refresher Class at the Upper Saddle River Volunteer Ambulance Corps. 


Upper Saddle River Ambulance Corps

This course will start       and will end      , from 8:00 AM to 5:00 PM each session.  All sessions must be attended.  Students must show a valid, current CPR Professional Rescuer level certification.  The cost will be $150 for the Refresher class.
